Required Forms for High School Band Members
Below you will find the FOUR forms you have to complete and submit before band camp:
Physical (must be completed by a doctor every year, and forms must be submitted to the athletics office at CHS)
Rank One (an online form website that must be completed before camp)

You must also complete all payments before camp.

All band members, incoming grades 9-12, are required to submit a current physical each year. Physicals must be submitted to the athletic department at CHS.
