Get Ready for Camp - Required Forms for High School Band Members

Below you will find the FOUR forms you have to complete and submit before band camp:

Physical (must be completed by a doctor every year, and forms must be submitted to the athletics office at CHS)
Rank One (an online form website that must be completed before camp)
UIL Acknowledgment Form
Band Policy and Student Pick Up Form

You must also complete all payments before camp.

Pay HS Band Fees

Financial Obligations

price for wind and percussion members is $260
price for guard is $350
all members pay a $120 deposit by May 15 this is mandatory, as we must pay a deposit for band uniforms
remaining $140 is due by July 20 (remaining $230 for guard)
fees include show uniform, food before football games, and a band t-shirt

Perennial Needs (required, purchase as needed)

All first time members must purchase the all black dry-fit shirt and shoes (moving to a new shoe design). Returning members have the option of purchasing a new shirt and shoes or using the ones they previously purchased.
Large water jug for games and rehearsals (no water bottles provided)
Lyre and flip folder
Long black socks
Solid black athletic shorts

Physicals

All band members, incoming grades 9-12, are required to submit a current physical each year. Physicals must be submitted to the athletic department at CHS.

Rank One forms must be completed online. Blank fields will not be accepted. Please enter N/A on anything that does not apply. There are several sections to complete.
