Physical Form - This form will have to be filled
out by your doctor and filed with the athletic
department.
Medication Form - This form must be completed and
submitted to Ms. Hoffman before we leave for band camp
or any overnight band trip.
Permission Slip - This form must be completed and
submitted before we leave for camp.
